MobilityPLUS subscription trip request

Use this form to make one request for a subscription trip.

Subscription trips are ongoing rides occurring:

  • On the same day/days
  • At the same time
  • To the same location
  • For a minimum of three weeks.

You only need to set up the trip schedule once.

All subscription trips, including trips to dialysis appointments, will be cancelled on statutory holidays (not including Remembrance Day or Easter Monday).

Please call 519-744-2241 seven days in advance to rebook your trip.

GRT may suspend the subscription privilege if rides are not taken regularly.

Select a 30-minute pick-up window or your appointment time

30-minute pick-up window: The vehicle will arrive at your pick-up location within the time that you select, e.g. 10:00-10:30 a.m.

Appointment time: The vehicle will be scheduled to drop you off prior to your appointment time. Please check the business hours and ensure that the building is open prior to your appointment time, so you can enter the building when you are dropped off.
